    What if the secret to sharper decisions lies not in your strategy, but in your surroundings? We spend much of our leadership energy on strategy and systems. Yet the physical environment we work in...the light, the noise and the temperature shapes our ability to think clearly and make good decisions. Researchers note that exposure to light not only governs vision but also influences alertness, cognition and mood. Bright light reduces sleepiness and improves neuro behavioural performance. Conversely, high levels of noise, particularly irrelevant speech, diminish cognitive performance more than temperature. In one study, researchers observed optimal cognitive performance at a moderate temperature with noise levels around 55 dB. I saw this play out when we refreshed the back office of a restaurant I was overseeing. The team had been working under harsh fluorescent lights and constant background chatter from the kitchen. People were tired, mistakes crept in and tensions rose. After reading about the effects of the environment, we replaced the lighting with softer, brighter bulbs, opened blinds to let natural light in and set up a quiet area away from the busiest machines. Within days, the mood lifted. Staff reported feeling more alert and less stressed. For leaders looking to harness the environment, here are a few considerations: 1. Let in the light. Where possible, increase exposure to daylight or use bright lighting. Evidence suggests that this helps maintain alertness and reduces sleepiness. 2. Control noise. Background chatter and irrelevant speech can impair concentration. Aim for moderate noise levels and quiet zones if your space allows. 3. Mind the temperature. Studies have found that cognitive performance peaks at moderate temperatures and falls when rooms are too cold or too hot. 4. Observe and adjust. Walk through your workspace at different times. Notice where people seem energised or drained. By managing light, sound and comfort, we give ourselves and our teams a better platform to perform.     Are you a morning person or night owl? Plot twist: it doesn't matter nearly as much as people think. A massive study of 26,000+ people just revealed something wild. Yes, night owls scored slightly higher on cognitive tests than morning people. BUT here's what actually matters from this research: Sleep duration absolutely crushed chronotype as a predictor of cognitive performance. Let me repeat: in terms of performance the next day, whether you're a morning or evening person doesn't matter as much as how much sleep you get. People sleeping 7-9 hours consistently outperformed everyone else, regardless of whether they were early birds or night owls. Of course, your biological clock is real. Honor it when you can.     Sleep is the brain’s most powerful performance tool, and most people treat it like a negotiable expense. Neuroscience is blunt: when you cut sleep, the brain shifts into survival mode. Astrocytes prune more synapses. Microglia stay activated. The glymphatic “night shift” that clears waste runs poorly. You don’t just feel tired. You lose clarity, memory consolidation, and emotional control. Decisions get riskier. Empathy gets thinner. Creativity shrinks. It’s not hours you’re sacrificing. It’s executive function. High performance isn’t willpower, it’s architecture. The brain thrives in rhythm, not chaos. Try this for 7 days: • Wake at the same time daily (weekends too).     Sleep apnea is often brushed off as “just snoring” or feeling tired. It’s far more serious than that. This condition causes repeated pauses in breathing during sleep, which triggers low oxygen levels, interrupted rest, and chronic stress on the body. Over time, sleep apnea can lead to: • High blood pressure • Irregular heart rhythms • Heart failure • Stroke • Type 2 diabetes • Depression • Cognitive decline, including Alzheimer’s The heart, especially, pays the price. When oxygen drops and the body jolts awake repeatedly, the cardiovascular system is constantly strained. Over the years, this can lead to structural changes in the heart and increase the risk of major events like heart attacks and strokes.     A 2009 laboratory study (PMID: 19240794) found that 5 nights of sleep restriction (4 hours/night) elevated CRP, a marker and mediator of cardiovascular inflammation, by 145%, with levels increasing further to 231% even after 2 nights of 8-hour recovery sleep. Study: - Participants: 19 healthy men, ages 19–29. - Design: Controlled sleep study simulating a workweek (5 nights sleep .restriction, 2 nights recovery). - Groups: Sleep-restricted (n=13) vs. control (n=6; 8 hrs/night throughout). - Primary outcomes: CRP levels, cytokine expression, immune cell activity. - Cytokines measured: IL-6, IL-1β, IL-17. - Other metrics: Heart rate, salivary cortisol, blood pressure, PBMC proliferation. Findings: ✅ CRP increased by 145% after sleep restriction, and 231% after 2 nights of recovery sleep. ✅ IL-6 (↑63%) and IL-1β (↑37%) remained elevated post-recovery. ✅ IL-17 stayed elevated at the protein level even after recovery. ✅ Heart rate also increased, while cortisol and blood pressure remained stable. ✅ Suggests sustained immune activation and inflammatory signaling. Limitations: 🔹 Small sample size; young, healthy male participants only. 🔹 Recovery period (2 nights) may be insufficient to reverse immune activation. 🔹 Study focused on acute inflammation; long-term cardiovascular outcomes were not assessed. 🔹 Results may not generalize to women, older adults, or clinical populations. Persistent CRP elevation, even after resuming normal sleep, may represent a delayed immune “cooldown,” suggesting risk of chronic low-grade inflammation from repeated sleep loss.

    The brain has its own waste clearance system that most people, including many clinicians, don't think about nearly enough. It's called the glymphatic system. It works by circulating cerebrospinal fluid through brain tissue to flush out metabolic waste, including the proteins most strongly implicated in neurodegeneration: amyloid-β, tau, and α-synuclein. The mechanics depend on aquaporin-4 (AQP4) water channels on perivascular astrocytic endfeet. When properly polarized, fluid exchange between CSF and the brain's interstitial compartment is efficient. When that breaks down, so does clearance, and that's one feature we see in Alzheimer's and Parkinson's disease. Deep slow-wave sleep is the most recognized driver of glymphatic function, but it's not the only one. Three factors that get less attention: 1. Cardiovascular health. Hypertension, insulin resistance, and dyslipidemia lead to cerebrovascular stiffness and small vessel disease, which impairs glymphatic function. As I always say, what's good for the heart is good for the brain. 2. Respiratory patterns. Each respiratory cycle drives pulsatile CSF movement through the perivascular network. In obstructive sleep apnea, fragmented sleep combined with disrupted pressure dynamics may significantly impair glymphatic transport. Emerging data suggests CPAP therapy may restore CSF flow yet another compelling reason to manage OSA aggressively in patients at neurodegenerative risk. 3. Body position. Preclinical studies suggest lateral sleep position supports more efficient glymphatic transport compared to supine or prone, through changes in perivascular geometry and CSF flow dynamics. Much of this research is in animal models. Human translational work is ongoing, but worth following closely. One honest caveat: studying the glymphatic system in humans is difficult. Advanced MRI techniques measure structural properties of the perivascular network, not actual clearance kinetics of amyloid-β or tau. The biology is compelling, I find it genuinely exciting, but we're still working to understand how these mechanisms translate to human brain health. What seems clear is that vascular health, breathing, and sleep quality all influence whether the brain can efficiently clear the proteins associated with neurodegeneration. The more I read about this system, the harder it is to overlook these factors in a comprehensive brain health assessment. #preventiveneurology
